Changing reporting name on class accountGuidelines For Changing Reporting Name of Class for Tax ID Number Instructions should only be used if your Class has already been assigned an EIN. An EIN is permanent for as long as the Class exists. The only things that change are the mailing address and the "care of" person. The change process is simple: 1. Write a letter referencing the EIN number. 2. Inform that the "care of person" isted on Line 3 should be changed. 3. Inform that the mailing address is now XXXX (be sure to include county and state). 4. Include your daytime phone number. 5. Sign and date and provide a PRINTED name and your title as well. 6. Mail to: Internal Revenue Service NOTE: This will vary from region to region.
